History

Heart rhythm medicine like amiodarone (Cordarone, Pacerone), disopyramide (Norpace), dofetilide (Tikosyn), procainamide (Procan, Pronestyl), ibutilide (Corvert), quinidine (Quinidex), and sotalol (Betapace).
Problem of kidney, seizures or epilepsy, low potassium levels in blood (hypokalemia), diabetes, or a Long QT syndrome history in the family should be reported to the doctor as well.
